چکیده

Purpose of review Increased total body fat with truncal redistribution is common in antiretroviral therapy (ART)-controlled persons living HIV(PLWH), leading to insulin resistance, prediabetes/diabetes and dyslipidaemia. We address these topics here. Recent findings Most antiretrovirals are associated gain trunk fat, including visceral adipose tissue (VAT). Protease-inhibitors could inhibit white ability dissipate energy (i.e. beiging) favouring gain. Expansion VAT a pro-inflammatory profile linked the tryptophan-kynurenine pathway CD4+ subtypes. ART-associated increased (AT) quantity leads decreased AT density, resistance dyslipidaemia that be improved by lifestyle modifications. PLWH present high level regardless their treatment, higher prevalence prediabetes, but not diabetes, than noninfected persons. Otherwise, HbA1c values appear inaccurate diagnose PLWH. ART-related-dyslipidaemia characterized elevated LDL-C and/or triglycerides reduced HDL-C. Whereas treatment protease inhibitors generally results worsened lipid values, integrase-strand-transfer-inhibitors better profile. Tenofovir-alafenamide levels tenofovir-disoproxil-fumarate. Treatment LDL-C-dyslipidaemia benefit, statin-insufficiently controlled patients, from class proprotein-convertase-subtilsin-kenin-type-9 (PCSK-9) inhibitors. Summary Lifestyle modifications mandatory reduce improve dysglycaemia/dyslipidaemia. New drugs can efficiently control diabetes LDL-C-dyslipidaemia.

Dyslipidaemia associated with antiretroviral therapy in HIV-infected patients.

Highly active antiretroviral therapy (HAART) has had a significant impact on the natural history of human immunodeficiency virus (HIV) infection, leading to a remarkable decrease in its morbidity and mortality, but is frequently associated with clinical and metabolic complications. Fat redistribution or lipodystrophy, hypertriglyceridaemia, hypercholesterolaemia, insulin resistance and diabetes...

Insulin Resistance and Diabetes Mellitus in HIV-Infected Patients Receiving Antiretroviral Therapy.

The introduction of highly active antiretroviral therapy (HAART) has dramatically improved the long-term prognosis of human immunodeficiency virus (HIV)-infected patients, but several abnormalities of lipid and glucose metabolism have recently been reported with increasing frequency in patients receiving potent new antiretroviral combinations. Although a rare occurrence before the HAART era, in...

Rosuvastatin Worsens Insulin Resistance in HIV-Infected Adults on Antiretroviral Therapy.

BACKGROUND Statins are associated with increased diabetes risk in large, human immunodeficiency virus (HIV)-uninfected cohorts; the impact of statins on insulin resistance or diabetes in HIV-infected persons has not been assessed within a randomized controlled study. METHODS HIV-infected participants on stable antiretroviral therapy with a low-density lipoprotein cholesterol level of ≤130 mg/...

Diabetes Mellitus in HIV-Infected Patients Receiving Antiretroviral Therapy

Background. There is little in the literature on HIV and diabetes mellitus (DM) in sub-Saharan Africa. Objective. To assess the characteristics of HIV and DM in patients receiving antiretroviral therapy (ART) in Botswana. Methods. A retrospective case-control study was conducted at 4 sites. Each HIV-infected patient with DM (n=48) was matched with 2 HIV-infected controls (n=108) by age (±2 year...

HIV Drug Resistance and Its Impact on Antiretroviral Therapy in Chinese HIV-Infected Patients

BACKGROUND Highly active antiretroviral therapy (HAART) has significantly decreased mortality among Chinese HIV patients. However, emerging HIV drug resistance (HIVDR) poses a growing threat to the long-term success and durability of HAART. METHODS Three cross-sectional surveys were conducted across the country from 2004 to 2006, respectively. Patients completed a questionnaire and provided b...
